It depends what country you are wishing to visit. I know that many of the stately homes and castles in the UK have retro fitted disabled/wheelchair access, if only by having elevators put in. Trains will help you on by bringing out ramps too, but it is better if you let them know in advance.
Some places would be off
Some places would be off limits to wheelchair users for sure. There are older buildings that are under protection orders, that you might not be able to manage the stairs. Other disabilities are better catered for by allowing working dogs inside, providing hearing equipment (t-loops for hearing aid users and so on) in theatres, banks and the likes.

Yeah, there isn't a place in operation that I would think would get away with not allowing working dogs. As far as wheelchair access is concerned though, you'd be best to call ahead.

I think it may also depend on your type of disability, like someone had mentioned the stairs. I think most tour operators would be able to take care of most needs, just depends what exactly would need done. :)
